Job Description

Position Summary:

Circle of Care St. Louis is seeking a dedicated and compassionate Transition Coordinator to assist individuals transitioning from skilled nursing facilities back into the community. This role is crucial in promoting the health, safety, and independence of our participants.

Essential Duties:

  • Conduct assessments and develop personalized transition plans for nursing facility residents.
  • Coordinate housing, personal care, medical, and community support services for participants.
  • Schedule and attend discharge meetings to ensure safe transitions.
  • Build and maintain relationships with healthcare providers, social services, and housing authorities.
  • Provide follow-up support, including home visits and monthly check-ins for the first year.
  • Collaborate with the Community Case Manager/Options Counselor to provide cohesive care.
  • Write detailed case notes documenting transition plans, services provided, and follow-up activities.
  • Ensure thorough documentation and compliance with training and background checks.

Education & Experience:

  • Ideal candidate: Recent college graduate with a background in health, nursing, or human services, or equivalent life experience.
  • At least four years of college or equivalent experience in a related field (e.g., health, nursing, or human services).
  • Experience working with seniors and individuals with disabilities is a plus but not required.
  • Strong executive functioning skills, including time management, organization, and the ability to write clear and detailed case notes.

Physical & Emotional Demands:

  • Ability to lift up to 35 lbs. for home setup.
  • Ability to travel daily to nursing homes and clients’ homes.
  • Ability to manage moderate stress during transitions and high call volumes.

Work Schedule:

  • Flexible starting schedule with 40 hours per week.

Compensation:

  • Starting pay: $19.50 - $20.00 per hour.
  • Candidates who do not stick to the compensation schedule will not be considered.
  • Reliable transportation required for daily travel.
