Job Description: 2nd Home Skilled Care is seeking a dedicated and experienced Registered Nurse (RN) for on-call or contract positions in Colorado Springs. The ideal candidate will have a strong background in home health care and be capable of providing high-quality patient care during recertification visits and start of care visits. Key Responsibilities: Conduct recertification visits and start of care assessments with patients.Develop, implement, and evaluate individualized patient care plans.Provide patient education and support to ensure understanding and adherence to care plans.Collaborate with physicians, patients, and their families to ensure comprehensive care.Maintain accurate and timely documentation of patient care and communication. Qualifications: Valid RN license in Colorado.Minimum of 1 year of experience in home health care.Strong clinical assessment and patient care skills.Excellent communication and interpersonal skills.Ability to work independently and manage time effectively.Reliable transportation and willingness to travel within the Colorado Springs area.
