KidzCare Pediatrics, PC is an NCQA certified Patient Centered Medical Home at 25 of our locations. The patient-centered medical home isa way of organizing primary care that emphasizes care coordination and communication to transform primary care into "what patients want it to be." Patient Centered Medical Homes lead to higher quality and lower costs and can improve patients’ and providers’ experience of care. We are also recognized as an AMH (Advanced Medical Home) organization with Care Management Support team members (RN’s, Social Workers and Care Coordinators).
